Welcome to K.Y.P.S.A.H. Pet Services!  We believe your pup will benefit from our services, and we look forward to watching him/her grow while in our care.

There are just a couple of things we need in order to get started:

1. Agreement Policies (PDF)
2. Copy of Vaccination Records

 

Easy Instructions as Follows

1. Please download, sign and send our agreement form (policies)–prior to the start of service.  If you would like to digitally sign the document and send it via email to us, that is completely doable as far as we’re concerned!
2. Please upload a copy of your pet’s most recent vaccination record.  If uploading the document doesn’t work for you and you would rather just give us a hard copy instead, no problem.
